If this sounds like you, Damar Services is looking for a talented and motivated Clinical Case Manager Coordinator to support a case load of clients on the diagnostic and evaluation units for high-risk clients. Attends mandatory staff meeting and other meetings as assigned. Represents clients in doctor’s appointments, psychiatric reviews, Court Reviews, Child and Family Team meetings, and IEP/Case Conferences by providing accurate information regarding the client’s treatment progress and areas of need. Responsible for direct supervision SAT/CNPCC BA Case Managers. Responsible for reviewing case management documentation of CANS, MHA, Stabilization Plans, Individual Crisis Management Plans (ICMPs), and biweekly reports. Responsible for a case load of high-risk children from SAT and CNPCC. Completes ICMP, CANS assessment, Mental Health Assessment, Stabilization Plan, and biweekly reports for assigned clients. Responsible for Independent Living assessments and execution of IL services, including weekly IL groups for children 14 and older. Trains staff over new client ICMPs and updated ICMPs for existing clients. Oversees Family Time scheduling and encourages family involvement. Completes and participates in all intakes and discharges for assigned clients. Acts as a liaison and consistently communicates with families, professionals, guardians and referring agencies to provide best outcomes for clients. Ensures that these communications are recorded in a timely manner and participates in weekly narrative communication. Completes and updates mandatory training and other licensure related items in a timely manner. Maintains documentation that is accurate and legible. Completes incident reports, including reporting any suspected exploitation, abuse, or neglect of a client. : What you need to be considered... Master’s Degree in Social Work, Psychology, or related human service area of study from an accredited program. Must have at least one year of experience in treating DD/ID population or specialized training and meet one of the previously listed degrees of education and/or accompanying years of experience. Additional license or specialized training the following preferred: LMHC, LCSW, LMFT, CSAYC or BCBA. Be at least 21 years old. Valid Indiana driver's license and good driving record. No felony convictions or other criminal offenses involving a dependent population. Successful completion of pre-employment drug screen and physical. Essential Job Functions: Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information. Vision: See in the normal visual range with or without correction. Hearing: Hear in the normal audio range with or without correction.
