Job Description
An existing opportunity to join the Discharge to Assess Team as a Clinical Support Worker/Therapy Assistant based at Furness General Hospital.
You will join a dedicated team of Occupational Therapists, Physiotherapists and Rehabilitation technical instructors to carry out discharge to assess home visits for patients leaving hospital. The role involves supporting the therapists to carry out assessments, making onwards referrals to other health and social care teams and carrying out some follow up visits to patient's homes as required.
All assessments will be within the patient’s home at point of discharge from hospital.
The ability to travel to a variety of venues throughout the community to deliver patient assessments within an identified locality.
Supporting Therapists to carry out functional assessments within the patients own home.
Delivery and fitting of assistive equipment as directed by the therapist
Supporting with moving and handling as required to meet patients needs
Communicating with patient, family and ward staff regarding outcomes of assessments and visits.
Making onwards referrals to appropriate social and health care providers as required using appropriate IT systems.
Supporting the wider Therapy Team at FGH as required. This may involve some ward based work.

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
