A vacancy has arisen for a Healthcare Assistant to join a community nursing team in Worthing.
The Healthcare Assistant will provide health care, and support under the direction and supervision of registered staff to patients living in the community. This includes assisting in the delivery of interventions, treating and discharging patients and reporting all changes in patient status to senior staff. The role also involves providing administrative support to the community team.
The Healthcare Assistant works in partnership with the primary health care team, social services, private and voluntary sector, addressing health promotion priorities both locally and nationally.
- To deliver holistic care when undertaking interventions to include pressure ulcer prevention advice and care, dementia awareness, observing nutrition and hydration status, falls risks, pain awareness and applying infection control standards.
- To deliver clinical interventions delegated by registrant, as appropriate to role and competency.
- To actively participate as a member of the multi-disciplinary team including contributing to meetings.
- To actively participate in the induction / orientation of new staff and students.

a) To undertake non-complex clinical procedures under the guidance of senior team members e.g.: venepuncture, patient specific sub-cutaneous injections , sample collection, blood pressure monitoring, medications, wound care, and other tasks according to training and competencies.
b) To recognise their responsibility and accountability in accepting the delegated task.
c) Empowering patients to regain their health and independence in collaboration with other members of the Multi professional Team.
d) Liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided.
e) To keep accurate, timely records according to Trust policy.
f) To escalate and report patient progress/ status on a shift by shift basis to senior team members.
g) To demonstrate flexibility and adaptability working across community teams according to service needs.
h) To commit to undertaking training and education to progress clinical skills as indicated by service development needs.
i) To respond effectively and actively to complaints and escalate appropriately following departmental policies.
j) To assist registered staff in care and treatment and participate in the review process.

l) To support carers in their role of carer. This may include communication and liaison with other agencies, including partnership agencies.
