Job Description

An exciting opportunity has arisen within our Learning Disability Services for a band 6 RNLD.

We are seeking an experienced, enthusiastic and motivated RNLD to take on the role of community nurse for people with learning disabilities.

The service covers the geographical area of Stoke-on-Trent, where the post holder will be work as part of the Community Learning Disability Health Team (CLDHT). As a band 6 within the team there will be expectation for the nurse to hold a complex caseload with responsibility for CPA and other legal frameworks. The successful applicant will have involvement with the transforming care agenda working with clients recently to returned to the area from hospitals, prisons and private care providers so understanding transitions an supporting clients going through significant life changes will be a significant part of the role

There will be an expectation that the role will include providing and delivering training

Supporting students and clinical supervising band 5 Co-workers and clinical support staff

We are seeking someone with the confidence to work autonomously and who has the resilience to cope with the demands of a busy but highly supportive team.

The service operates 9-5 Monday – Friday, and attracts all of the usual benefits that would be afforded from being part of the NHS.

Be a member of the CLDHT, demonstrating the ability to make decisions and influence the development of services.

Utilise advanced knowledge and skills to work with people with complex needs to meet defined objectives, reduce health inequalities by supporting people with mild, moderate or severe learning disabilities to access mainstream healthcare.

Ensure health professionals gain knowledge and skills in being able to provide healthcare that is responsive to their individual needs.

Participate in the development of an evolving learning disability service.

Will be required to teach staff including both students and qualified and unqualified team members.

North Staffordshire Combined Healthcare NHS Trust aims to provide accessible, non-stigmatising local services that contribute to the assessment, intervention, treatment, rehabilitation and enablement of people with learning disabilities, which promotes the prevention, early detection and treatment of health related issues for adults with learning disabilities in the community.

The Community Learning Disabilities Health Team (CLDHT) ensure that people with a learning disability gain full access to the health care they need, whether from a primary, acute, secondary or other providers. The service provides support by raising awareness through education and supporting professionals to identify their learning disability patients, and making any reasonable adjustments necessary as well as offering advice and guidance to professionals when working with someone with a learning disability.

Under the supervision of the Team Manager be accountable and responsible for a defined clinical caseload that reflects the priorities of the local service by ensuring the assessment of therapy needs, particularly in relation to:

To assist the wider CLDHT to support people with learning disabilities and their carers to navigate the wider NHS, either through direct support or advocacy at health appointments.

When required provide specialist support on a 1:1 basis to ensure understanding and implementation of consultant recommendations.

Complete an initial assessment of the physical health needs for people with learning disabilities who may have complex physical, psychological, behavioural or mental health needs.

Provide advice to other professionals on complex and/or sensitive care needs. This will principally be required in cases where there is no other Community Nurse involvement.

To promote the benefit and use of health action plans and deliver these to locally defined populations.
