Job Description

Key Responsibilities: Proactively support and coordinate the needs of patients undergoing treatment for Breast Cancer under the supervision of the clinical nurse specialist. Coordinate the transfer of care between service teams, or facilitation of service provision across interfaces of care- for example primary and secondary care. Refer and sign post to the clinical team as appropriate for decisions. Monitor, audit and report response times to inbound telephone calls.

Contribute to the holistic needs assessment, care plan, and review of patients in line with practitioner guidance, protocols and procedures. Evaluate outcomes of care support required for the patient with a Clinical Nurse Specialist. Triage incoming calls from patients and initiate appropriate responses based on clear protocols and frameworks. Make pre-planned outbound telephone calls to patients under the guidance of the Metastatic Breast Cancer Clinical Nurse Specialist.

Contribute to the clinical/quality management team reviews of any issues in relation to the patient pathway. Ensure the senior nursing team are aware of any symptoms which may need a more expert opinion. Ensure the senior nursing team are aware of any deterioration in psychological well being . Coordinate access to patient information and education resources for patients.

Undertake a focused mapping of the range of information/education support resources available in the local health and social care sectors. Support the delivery of information prescription delivery in consultation with health professionals and according to any agreed information pathways, including printing of information. Collaborate and work closely with local providers of validated information resources, users and user groups, including the Macmillan Cancer Information, The Living Well centre and other external Support groups/centres and registered providers. Contribute to the development of health and well-being events and information packs that support patients living with Metastatic Breast Cancer.

Communication Communicate directly through telephone or face to face contact with patients and carers demonstrating empathy, understanding, diplomacy, honesty and integrity. Demonstrate an awareness of practice and seek appropriate support and guidance. Communicate with referring centres either by email, telephone or letter in an appropriate manner to ensure that they are aware of the patients progress. Team Working Undertake specific organisational and administrative duties as required.

Participate in innovation and quality of healthcare by attending ward or department meetings and in projects, including audit and quality initiatives pertinent to the role. Take part in the orientation/induction of new Cancer Care Navigator and provide support and development to enable them to develop the skills and knowledge required for their role. Treat all patients and colleagues with respect in accordance with Trust values and Equality and Diversity Policy.
