Job Description

The post holder will provide an assessment of patients in this service to support the caseload of clients with Long COVID symptoms using evidence based and client-centred principles to assess, plan, implement and evaluate interventions.

The successful candidate will be expected to carry out all relevant forms of care, providing highly specialised advice and physiotherapeutic input for the treatment of this disease.

You will play a vital role in supporting the delivery of integrated, co-ordinated, comprehensive, seamless, person-centred care. As a member of the Tier 3 Long COVID Assessment & Treatment service Multidisciplinary Team (MDT) you will be required to make recommendations based on the principles of shared decision making for people living with or experiencing the impact of Long Covid symptoms and access Tier 3 Services.

To work in the community as a lone practitioner in line with the Trust’s Lone Worker Policy, providing chest physiotherapy in the client’s own home.

Care delivery according to changing health care needs and varying levels of complexity. To ensure that patients and their carers are involved in the planning, goal setting and reviewing of their treatment plans when ever possible. To provide self management plans as appropriate for all patients. To provide training and support for patients and their carers where appropriate. Empower others and demonstrate an ability to make effective clinical decisions based on recent evidence. Assessment and management of Bronchiectasis patient.

The post holder will work to support the Equality and Diversity agenda in the PCT ensuring engagement with the Directorate E&D lead and assisting them to achieve the E&D responsibilities in the Directorate.

Managerial:
To ensure that staff adhere to Health and Safety guidelines and maintain safe environments and working practices as appropriate.

Provide specialist, expert clinical knowledge to clinical colleagues, patients, and carers/relatives.

Undertake clinical supervision of physiotherapy colleagues on an individual or group basis.

Ensure the effective and efficient use of physical and financial resources.

Undertake clinical responsibility for a group of patients on a shift basis.

Monitor health, safety and security of self and others and promote best practice.

Work as an autonomous practitioner using evidence-based practice.

To facilitate pulmonary rehabilitation classes under supervision working towards independent after completion of competencies.

To assess patient’s suitability for pulmonary rehabilitation.

Leadership:
To provide professional supervision for Health Care Assistant, and students in the Community.

Perform comprehensive assessment of patient nursing and physiotherapy needs, plan, implement and evaluate care delivery according to changing health needs.

Manage a caseload of patients within the community setting, prioritising care as appropriate.

Participate in the development of services and assist the Team Co-ordinator to implement change. Clinical

Perform advanced clinical skills in assessment and/or diagnosis and prescribe treatment independently.

Deliver Specialist Physiotherapy treatment to patients referred into the LONG COVID Tier 3 service.

Work collaboratively with other professions and agencies to ensure patient needs are met especially in relation to ongoing care needs and discharge arrangements.

To perform a chest examination and conduct assessments for suitability for pulmonary rehabilitation.

Non-Clinical:
To maintain current written and/or electronic records of all interventions in accordance with professional and Trust standards.

To work and promote personal safety in a wide range of community settings as a lone practitioner in accordance with Trust lone worker policy.

To adhere to the Chartered Society of Physiotherapy codes of ethics and professional conduct.

To ensure all medical records are handled and stored in accordance with trust policy.

To maintain accurate recording of referral and caseload activity.

Responsibility to ensure equipment is checked and maintained in accordance with Trust policy and manufactures guidelines.

Training & Education:
Identify learning needs, plan, implement and evaluate programmes of education to meet identified need.

Participate in the education of pre and post registered nurses and curriculum development where appropriate.

Address specific health targets e.g., NICE Guidelines 2010 and National strategy COPD 2010 related to own area of practice through education.

Maintain accreditation for student training.

Develop own expert clinical knowledge.

Ensure own compliance with regard to mandatory training and PDR requirements.

Clinical Governance/Research and Audit.

Ensure compliance with policies, procedures and clinical guidelines for self and others participate in the development of same.

Continually monitor standards of care and participate in the improvement of care throughout benchmarking, audit, and research.

Promote patient and public involvement activities in the specialist area, leading to service improvement.

Ensure all risks are highlighted and reported in line with trust policy.

Promote people’s equality, diversity, and rights.
