Are you an experience Occupational Therapist that wants to be part of a new service that will make a difference to patients in their own home and avoid hospital admissions then we want to hear from you!

The Discharge to Assess service will be a seven day service working between the hours of 8am-6pm on a rota basis. We promote flexibility and are happy to support different working patterns.

We will support you with specialist and bespoke training to reach your potential and excel in your chosen career path. You will receive one to one clinical support, professional development and guidance.

You will provide specialist knowledge and clinical skills to deliver care within the D2A team and wider TCR service as required. As an Occupational Therapist in the D2A team you will have a fundamental role in the delivery of specialist assessment skills, knowledge and management of patients in the community whilst ensuring the delivery of high caliber care, and efficient use of facilities, manpower and other resources. Actively participating in improving the service the department/unit offers and will initiate and support future developments.

You will triage, assess, implement, plan and evaluate the delivery of direct patient care as an independent practitioner relevant to your Band.

Working as a practitioner within a multi-disciplinary / multi-agency service, you will facilitate timely discharge from an acute hospital or community bed and provide same day assessment and support planning to meet a person’s needs in the community.

You will provide and/or arrange appropriate support and interventions in the community working closely with colleagues across all sectors.

The Discharge to Assess team is a multi-skilled team made up of nurses, therapists, pharmacy, reablement, social work, support workers, and admin support. All clinical staff receive training to be multi-skilled to provide comprehensive initial assessments and develop relevant core competencies.

You will work flexibly to providing specialist Occupational therapy and rehabilitation support in the community as required, to provide the full discharge to assess service cover for up to 6 weeks of discharge from the hospital.
