- The post holder will be part of the Community Nursing Team, providing nursing care and will work in a variety of settings.
- This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served.
- Responsibility for team co-ordination will be assumed in the absence of the Operational Lead/Manager
- The post holder will be skilled in the assessment of care needs and sub-acute conditions and in the development, implementation and evaluation of care packages and if appropriate, rehabilitation packages.
- They will lead in organising the induction of clinical supervision of students and new team members and act as a role model to the team.
- This role expects the nurse to lead the defined team and the post holder will be in charge of that caseload with virtual and actual support from the Clinical Lead and Operational Lead under supervision.

To assess new service user referrals and care & support Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. The specialist care/support plans may include the following:
- Promoting independence/self-management and maintaining safety
- Increased functional ability
- Improvement in health and wellbeing including mental health
- Interventions in disease/condition management
- Prevention and reducing of health inequalities
- Admission avoidance including working with patients who frequently use the acute services
- Early supported discharge
