Modality Partnership Hull Division has a full-time vacancy for An Advanced Clinical Practitioner. The postholder will be a high performing qualified ACP, working across Hull Division delivering person centred, safe and effective patient care. Main duties of the job
The role is an all-rounded, clinical ACP role; the postholder will need to confidently evidence and utilise key skills such as ability to work on own initiative, ability to develop an evidence-based, personalised, integrated health and social care plan in partnership with the individual, carers, relatives and health care professionals, based on a full assessment of medical, nursing and social care needs, and demonstrate ability to act as an autonomous advanced practitioner providing a comprehensive clinical assessment service to patients/carers/families resulting in safe, appropriate clinical decisions. The postholder will work in collaboration with local divisions (including Divisional Boards) and the National Modality Business Support Team to deliver innovative ways of working to meet the strategic aims of the organisation.
The role is not a regular 9am-5pm job and is not a pure patient facing role; the job requires innovation, flexibility and commitment and the postholder will be required to work on quality improvement as outlined in the job specification, and work resourcefully as part of the team to ensure tasks are completed.

Person Specification: What are the minimum requirements all candidates need to meet?
Qualifications and Experience:

Qualifications - Essential
Registered first level general nurse
Recognised degree qualification in autonomous practice
Qualifications – Desirable
MSc or equivalent
Experience – Essential
Minimum of 5 years post registration experience
Minimum of 5 years recent primary and community nursing experience
Mentor/teaching qualification
Clinical supervision training and experience
Extended/supplementary prescriber
Experience Desirable
Experience of developing and maintaining positive and effective partnerships with partner organisations
Knowledge -Essential
Understanding of the local healthcare system, its priorities and challenges
Understanding of the national health and social care agenda and familiarity with emerging strategy
Understanding of the strategic direction of the ACP role and how this may develop over time
Advanced clinical practice skills in acute presentation of minor ailments.
Management of patients with complex needs.
