An exciting opportunity has arisen for an enthusiastic, innovative, flexible and well-motivated Occupational Therapist to join the East Integrated Rehabilitation Team based primarily within the Launceston Inpatient Therapy Team at Launceston Hospital.
Under the umbrella of the East Integrated Rehab Team we offer Occupational Therapy and Physiotherapy in the East Cornwall locality to individuals with a Primary Physical Health condition in a variety of ways:
- Launceston and Liskeard Community Hospitals – GP-led 18-bedded and 44-bedded rehabilitation units working as part of a team of Occupational Therapy, Physiotherapy, Nursing and Therapy support workers.
- Community Rehabilitation Team – within the person’s home environment and intervention targeted at a wide variety of conditions, eg long-term, neurological, orthopaedic, falls, frailty.
- Intermediate Care – A more urgent service and working closely with the Home First/Early intervention team in the most appropriate environment for the individual
- East Community Orthopaedic Service Team – A community Therapy team for patients undergoing elective hip and knee replacement surgery in Plymouth.
- The Integrated Therapy team works closely with Specialist Therapy teams and is part of the East Cornwall Locality integrated Care Team working with Community Nursing, Social Care, Volunteer and Mental Health teams
The current service contract is operational Monday to Friday but seven day working may be a requirement of this post in the future. It may be that on occasions and as a requirement of this post that the post holder works flexibly over the East Integrated Rehab Team area. We actively support and encourge professional development with a variety of CPD opportunities offered
- To manage a defined caseload of patients with complex needs within the Integrated Therapy environment using evidence based practice, Occupational Therapy process and patient-centred principles
- To assess, plan, implement and evaluate interventions as an autonomous practitioner using independence of judgement
- To provide leadership for support staff through supervision, training and appraisal
- To participate in the planning and development of Occupational Therapy services within the Integrated Therapy Service
