Job Description

An exciting opportunity has arisen for Health Care Assistants to join our Staff Bank across The Hillingdon Hospitals NHS Foundation Trust.

We are seeking enthusiastic and caring individuals, who would enjoy the challenge of working as a health care assistant in a critical care environment. The role is to provide support to the trained nurses in caring for their patients.

A large part of the role involves maintaining high standards to ensure infection control standards are met. It is a excellent opportunity for individuals wishing to develop their interpersonal, communication and caring skills.

The healthcare assistant / support worker will Provide patient centred, compassionate care at all times and advocate for the patient

The healthcare assistant / support worker is a member of the team that delivers clinical care under the supervision of a registered nurse. Tasks are delegated by the registered nurse.

The post holder is responsible for ensuring that patients are cared for in a safe environment and that privacy and dignity is maintained at all times.

The post holder will provide direct care and provide verbal and written reports.

To actively seek the views of patients / carers on their understanding of their care needs and develop a relationship to ensure the patients experience is positive.

The Healthcare Assistant is required to successfully complete all 15 standards within the Induction Care Certificate and associated workbook within the set timeframe of 4 weeks (maximum 12 weeks).

To assist the registered nurse / professional in the delivery of patient care.

To communicate with patients, in such a way, that respects and takes into account the patient’s level of understanding.

To assist with the preparation and delivery of meals and beverages, assisting patients as requested.

Adhere to strict compliance with Trust Infection Control policies and procedures.

To participate in the admission process of patients as directed by the registered nurse.

To assist the registered nurse / professional in the delivery of patient care – pre, peri and post procedures, as required.

To communicate effectively with patients / carers to ensure patients are kept well informed of any changes or delays to appointments / investigations.

To be responsible for undertaking and recording blood pressure, pulse, temperature, respiration, urine analysis, weight, height, reporting any abnormalities to the nurse / professional in charge.

To help patients within the parameters of their level of mobility, using equipment as required, ensuring risks are minimised.

Help patients to look after themselves and maintain their personal hygiene whilst ensuring their privacy and dignity is maintained. To wash and bathe the patient, in bed or in the bathroom. To care for patient’s mouths, hair nails and pressure areas. To help patients dress and undress.

To help patients to be as comfortable as possible during their stay and to ensure that they are able to rest, providing a safe, controlled environment.

To assist and enable patients to access clean and safe toilet facilities. To care for incontinent patients, following the plan of care. Maintain necessary documentation in relation to fluid balance.

Keep patients bed space tidy, cleaning the bed and locker and any other furniture, as necessary.

To assist and help the trained nurse in supporting patients and carers during their visit, as appropriate. To provide on-going support to patients and significant others.

To help patients pack and unpack clothing on admission, transfer or discharge. To ensure that property is documented and stored as per Trust policy.

Assist the trained staff with the performance of last offices of care.

To escort patients to other departments and ensure post holder is sufficiently informed to respond to questions from other departments on the condition of the patient.

To find relevant reports e.g. pathology results on the computer and pass these to the appropriate professional. To file appropriate notes and reports and send information to others i.e. other departments.

To monitor and top up medical stock, linen supplies.

Identify aggressive / abusive behaviour and obtain help from senior members of staff.

To act as a chaperone as requested.

To enable patients / carers to find out about services available and support them when using these services, by liasing with the registered nurse and other members of the multi-disciplinary team.

To encourage communication and assist patient / carer when required by discussing their plan of care with them and following instructions from specific specialists i.e. Speech and Language Therapists.

To act in accordance with the Data Protection Act, Caldicott and Freedom of Information Act

To adhere toALLtrust policies and guidelines and ensure that own practice in relation to Infection Control is exemplary.

To be fully conversant with the Trust Policy on Child Protection issues and to report any observation / concern to the nurse leading the shift.

To ensure that training has been received before undertaking any interventional task requested by a registered nurse / professional.

To ensure that mandatory training is planned and attended as per Trust policy.

It is an expectation to attend the Essential Care Certificate and associated qualifications.

Attend skills workshop update session.

To participate in activities relating to local audit within the clinical environment.

To contribute to the delivery of clinical governance plans as requested.

To report any incidents in line with the Incident Reporting Policy and participate in any investigation.

Be aware of and action emergency procedures in the event of a cardiac arrest and a major incident.

Undertake training as required to ensure effective, efficient use of patient management system and working within hospital guidelines.
