Coventry Children’s Services has a specialist Children Disability Team (CDT) which provides support to children and young people up to the age of 18 who have a lifelong learning/physical disability.
The CDT is passionate about working with children, families and other professionals to achieve the best outcomes for vulnerable children. The team undertake children and family assessments and works with children in need, children subject to child protection plans and looked after children. A multi-disciplinary approach is modelled in the team in partnership with both universal and mainstream services to support children, young people and their families.
What is the job role?
You will be responsible for leading and supporting plans for children with complex needs and disabilities and their families. Children and families will be allocated by a Senior Practitioner or Team Manager, which may include Carer's assessments, including planned intensive interventions, family support, parenting support with children, young people and their families subject to children in need plans, Child Protection Plans, and looked after children.
The role requires you to work in partnership and empower children and young people with complex needs and disabilities, and their families to sustain positive behaviour changes. Act as a Key Worker with case management responsibilities to coordinate and plan integrated and intensive family support.
This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
Who are we looking for?
We are looking for an ambitious, professional, conscientious person with experience working with children and families to join our team.
You will need to have worked with children and young people with complex needs and disabilities, be knowledgeable about the challenges experienced by families as a whole and be skilled in completing appropriate assessments and support.
You will also need to be able to deliver individual and group work programmes to children, young people, and their families, using a range of communication aids and methods, in a range of settings.
We are looking for a team player, working with colleagues in a range of disciplines, sharing knowledge, and provides support and advice to deliver the best possible outcomes for local children and their families.
