The successful candidate will be passionate about older people’s care and about furthering and developing high quality, evidence based care across the interface of acute and community services. This post will be suitable for candidates to apply for if they are either a fully qualified Advanced Clinical Practitioner (ACP), already within their ACP training pathway or looking to begin their training.
If you undertake a Trainee ACP pathway - This will be in line with Health Education England requirements. The two routes that are available would be an MSc or portfolio routes. These will be discussed at interview.
In addition, you will follow a robust and structured ACP pathway which will lead to qualification and accreditation as an Advanced Clinical Practitioner, following completion of the training pathway.
The requirements of this Job Description are aimed at fully qualified Band 8A ACPs which a Trainee will be expected to meet after completing their training.

- Ensure every person receives personalised care starting with a ‘What matters to you’ conversation and including shared decision making at the heart of all decisions
- Assess, diagnose, plan, implement and evaluate treatments and interventions for older people across the Trust/division/specialty
- Under-take advanced physical and mental health examination as indicated by the patient’s condition; analysing complex clinical signs and investigating results to consider differential diagnoses and diagnose conditions, initiating any treatment as required
- Perform accurate clinical procedures appropriate to area of practice and within scope and competence of practice, from undertaking history taking and examination of the patient, including further invasive testing, diagnostics and treatments requiring highly developed skills and precision, for example completing Comprehensive Geriatric assessments and diagnosis when there are multiple co-morbidities
- Request investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, x-rays and computed tomography (CT) scans in accordance with IR(ME)R regulations and as local policy allows. Interpret and act on investigations results planning, and managing clinical care and interventions as required.
The Healthcare for Older People (HOP) team is a hard-working, dedicated and accommodating team.
Consultant numbers over the last 15 years have expanded from 3 WTE to 9.1 WTE (excluding stroke consultants’ numbers) and have also diversified with the addition of a Specialty Doctor, a Consultant Nurse and an Advanced Clinical Practitioner in Frailty and three Specialist Nurses in Parkinson’s Disease and Movement Disorder.
Our specialty base currently includes two 28-bedded acute medical wards specialising in healthcare for older people. We have a separate 28-bedded acute stroke ward with 2.9 WTE Consultant Stroke Physicians. In one of our community hospitals we have a 15-bedded stroke rehabilitation ward. Our rehabilitation beds are based in our 4 community hospitals and Intermediate Care beds within our local care homes.
We also provide an Orthogeriatric service, Parkinson’s Disease & Movement Disorder service and rapid access clinics. In the last 6 months we have taken over the responsibility for providing medical cover to the 18 beds at Totnes Community Hospital working closely with the team there to develop a sustainable future model that will also support the training of medical students, foundation doctors and registrars.
