Job Description

Main Duties To undertake health needs assessments and screening for children as directed by the named or delegating Health Visitor and ensure prompt feedback and escalation of concerns as required Participate in the planning, delivery and evaluation of health promotion activities as part of an integrated team for individuals, families groups and communities utilising evidenced based practice. Contribute to the BFS early help offer supporting families to access additional support as required in line with the early help framework. Contribute to the safeguarding of children at risk in line with appropriate Safeguarding policies and procedures. Participate in and promote the childhood immunisation programs as appropriate.

Participate in the Trust staff nurse development program and other relatedTrust based learning opportunities Work within the NMC code of professional conduct. Support Health Visiting Teams in the management of children with additional medical needs, completion of Educational & Health care plans and preparing/supporting families for transition to education. Support and deliver evidence based packages of care for children in line with the BFS Standard Operational Procedures for Health Visiting and clinical pathways such as the premature care baby pathway and children identified as having Special Educational Needs. Liaison with other Multi Disciplinary Teams to ensure effective communication across the teams to support the HV oversight and co-ordination of care.

To support the Health Visting and Early Years teams to deliver Early Help intervention and complete Early Help Plans as required. To keep accurate and contemporaneous electronic patients records for children and families in line with NMC requirements and the Trusts Record Keeping policies and guidelines. To convey information, of a complex nature, to other professionals and to clients with well developed oral communication skills. This will include negotiating, showing empathy and reassurance, to have the ability to gain co-operation of others when needed and overcome barriers to understanding.

Undertake clinical audits as directed by Health Visitor/Health Visitor Team Leader, supporting actions plans according to findings. Interpret difficult / challenging situations, take prompt action and have the ability to diffuse potentially aggressive or threatening situations. Report any incidence that gives rise to concern. Work with and support families through difficult family situations such as illness and bereavement.

To plan, organise and run clinics to support the health and developmental needs of children as required Ensure data is inputted onto a data collection system accurately and on time as required by the Trust. To deliver care in a variety of settings, childrens centres, clinics, GP surgeries and clients homes. The latter requires lone working within the community setting. To ensure effective working relationships and communicate with the BFS team and other professionals.

To maintain and continually update professional knowledge and skills. Attend mandatory training and relevant updates training sessions as required by the Trust. To co-operate and assist in research and survey programmes relating to the client group. To participate in the teaching/training and assessment of support staff within the cluster, and in the clinical placement of all learners e.g.

pre-registration. Management and Leadership Responsibilities To comply with Birmingham Forward Steps policies, procedures and guidelines and with relevant statutory and/or Department of Health directives To attend and participate in staff meetings, integrated allocation meetings and other meetings as directed. To supervise, organise and monitor appropriate staff, as necessary, to ensure effective service delivery.
